Abstract
Objectives : Based on singleton birth certificate data from National Statistical Office for 1998, 2003 and 2009, this study aimed at analyzing the secular trends of gestational length distribution in Korea.

Results : From 1998 to 2009, the gestational length distribution in singleton births shifted toward shorter gestation distribution (38-39 weeks), and mean gestational length decreased from 39.4 weeks to 39.1 weeks during the periods. The proportion of singleton birth at 40 and more weeks gestation decreased, accompanied by an increase in those at less than 39 weeks. Between 1998 and 2009, the late preterm birth rate increased more than 1.5 times (from 2.3 percent to 3.4 percent), and post term birth rate decreased from 2.1 percent to 0.4 percent (odds ratio: 0.17, 95% confidence interval: 0.16-0.18). The term birth rate was unchanged at 95 percent.

Conclusion : The rise in preterm birth and decrease in mean gestational length during the 1998-2009 may be
related to the increasing use of induction labor and cesarean delivery. In future studies for secular trend of gestational length, it could be implemented nationally to monitor the way labor and delivery, such as spontaneous delivery, induction labor and cesarean delivery.
